The first human studies also exist which use positron emission tomography to suggest that receptor and transporter availability in the synaptic cleft is dependent on sex hormones. Most of these studies are based on cross-sectional studies and therefore do not allow any conclusions to be drawn about the causality of the observed dependency. The study

Transsexual subjects of both sexes were examined, who were measured three times in a longitudinal study design using PET and the radioligand [11C] DASB to quantify the 5-HTT concentration in the synaptic cleft. Blood was taken at each PET appointment to determine the plasma levels of estrogen and testosterone. Furthermore, female and male control subjects were measured once and a subset of the male control subjects was measured twice with PET and [11C] DASB. As expected by the researchers, after four weeks of testosterone therapy with FzMT, there was a significant increase in plasma testosterone in the male reference range, which was even more pronounced after four months of therapy. In contrast, the testosterone concentration in plasma from MzFT after four weeks of anti-androgen therapy in the female reference range, where they remained after four months of therapy.

Analysis of the PET data showed that the 5-HTT binding potential after four weeks – and even more so after four months – of testosterone therapy with FzMT was significantly increased in contrast to the first measurement in several subcortical regions (see figure). This increase was particularly evident in the almond kernel and in the striatum. Furthermore, a significant positive correlation between the increase in testosterone concentration in plasma and the increase in 5-HTT binding in these regions was found, a finding that underpins the postulated causal relationship between testosterone and transporter density. In contrast, there was no significant difference in MFT after four weeks of antiandrogen and estrogen therapy; however, there was a significant reduction in 5-HTT binding in the striatum as well as in the cingular cortex and the island after four months of therapy. Interestingly, however, there was a significantly negative relationship between the decrease in 5-HTT binding in these regions and the increase in estrogen levels.

Implications

The results of this study suggest that testosterone increases the density of serotonin transporters on the cell surface in subcortical brain structures. These findings are in line with rat studies, which found increased 5-HTT density and 5-HTT messenger RNA after testosterone administration. Since testosterone is largely converted to estrogen in the brain via the aromatase enzyme and many functions of testosterone affect the function and structure of the brain in this way, it can be assumed that the testosterone effect observed here can also be explained by this mechanism.

This is also supported by the result that the increase in estrogen in MFT had an apparently “protective” effect on the decline in serotonin transporters. Furthermore, it can be assumed that testosterone (and the estrogen that arises from it in the brain) not only act directly on the expression of the serotonin transporter through the binding of nuclear receptors but also indirectly increases the expression of the serotonin transporter through the increased release of serotonin. This may explain why testosterone therapy has a positive effect on mood in hypogonadal men.

The serotonin transporter, in turn, is responsible for the uptake of extracellular serotonin, and an increased 5-HTT density suggests an increased serotonin uptake; the expression of the serotonin transporter itself is strongly regulated by the serotonin content in the synaptic cleft. A low 5-HTT density can thus be interpreted as an expression of reduced serotonergic neurotransmission, while an increased 5-HTT binding potential reflects higher serotonin concentrations in the synaptic cleft.

The men were 60 years or older and received either the hormone or a dummy drug. First and foremost, the researchers wanted to find out how adolescent testosterone levels affect older men’s vessels. Previous studies had vaguely indicated that high levels of testosterone in the blood could have a positive effect on developing atherosclerosis. At the same time, other studies suggested that the hormone supplied increased the risk of a heart attack.

Bhasin and his team were at least able to rule out the positive effect on the arteries through their study. At the same time, however, they again found that testosterone also did not reduce men’s pleasure and potency problems. The preparations had no overall effect on the general quality of life. In another study, part of the research team also showed that age-related problems with ejaculation could not be remedied by testosterone.

The only detectable effects were, therefore, the side effects of the substitution: In the testosterone group, the level of prostate-specific antigen (PSA) in the blood rose significantly, in some cases above the level used in the early detection of prostate cancer as a reason for a biopsy. In fact, testosterone can stimulate the growth of small, yet undiscovered tumors. The so-called hematocrit also climbed in this group. It describes the cell density in the blood, i.e. its toughness. The hematocrit also increases the risk of blood clots that can clog arteries and lead to strokes.

Contrary to scientific advice, the number of treatments is increasing

Especially healthy men should, therefore, avoid substitution despite the physical burden of old age. Serious andro- and urologists have been warning their clients for more than ten years not to talk themselves into illness and to have them treated with expensive testosterone preparations. Nevertheless, interest in unnecessary therapy does not wane. In the United States, the number of testosterone treatments rose from 1.3 to 2.3 million between 2009 and 2013. In Sweden, the number of men treated almost doubled within eight years.

And in Germany, too, the requests for a substitution of the supposedly missing hormone according to the German Society for Endocrinology are steadily increasing – although it is difficult to say to what extent. However, experts assume that the consumption of testosterone in Germany has doubled in the past 15 years. The manufacturers of the preparations can, therefore, rely on it: The fear of not only getting old but also feeling old remains. In fact, it would be too nice if a plaster would help.

Studies of this kind have consolidated the myth of the male testosterone. Such studies are not very meaningful for closer inspection. Because, as a rule, scientists measure the testosterone concentration of the study participants in the blood. The amount present in the brain, which is crucial for the psyche and behavior, can vary considerably. In addition, cause and effect should not be confused: researchers showed hockey players the video of a game that their team won. As a result, her testosterone level rose. “Higher testosterone levels can be the result of aggression instead of the other way around,” presumes the neurobiologist Christoph Eisenegger from the University of Vienna.

The sex hormone is ideal for accelerating the regeneration of tired muscles. Kurt Moosburger, sports and nutritionist and specialist in internal medicine from Tyrol, believes that testosterone is therefore almost essential in modern cycling.

“You can complete a difficult Alpine stage without doping,” he said in an interview with the DPA news agency. But then the muscles were burned out. “Depending on the state of your training, you need one to three days to regenerate.”

According to Moosburger, testosterone and the growth hormone HGH can accelerate regeneration: “Both are endogenous and therefore natural substances. They help to build muscle as well as to regenerate the muscles.”

Fritz Sörgel from the Nuremberg Institute for Biomedical and Pharmaceutical Research confirms the regenerative effect in an interview with SPIEGEL ONLINE: “Testosterone is used precisely for this purpose in medicine, for example in HIV or tumor patients.”

According to Moosburger, testosterone can be administered in such a way that it can hardly be detected. “You stick a commercially available testosterone patch, such as that used for hormone replacement therapy in men, on the scrotum and leave it there for about six hours.” The low dose is not enough to produce a positive urine test, but the body actually feels faster recovery.

Sörgel suspects that Landis miscalculated in his presumed testosterone administration and was therefore noticed during the doping test.

However, hormone administration is not just about faster regeneration: testosterone also promotes muscle growth, stimulates blood formation and strengthens the feeling of power and aggressiveness. Because of these performance-enhancing effects, the hormone is classified as an anabolic steroid, the administration is considered doping.

Sörgel believes that the psychological effects of testosterone are crucial for competition: “‘I have unleashed afterward’ – athletes report this repeatedly after taking prohibited steroids.”

Friedrich Jockenhövel, a chief physician at the Evangelische Krankenhaus Herne, believes that tour participants would generally benefit from a hormone intake. “It is a well-known phenomenon that the testosterone production of the testicles decreases with continuous exercise,” he told SPIEGEL ONLINE. For drivers without hormone addition, the level drops during a stage and overall during the tour. “When an athlete is treated, his testosterone level is more balanced and certainly higher than that of other drivers,” said Jockenhövel.

Testosterone injection for three weeks

Jockenhövel is well versed in testosterone in top sports. He has several competitive athletes as patients who regularly receive testosterone after chemotherapy. “In 80 percent of those treated with chemotherapy, the testicles are so damaged afterward that hormones have to be given permanently,” said the specialist.

The hormone could best be given as an intramuscular injection into the buttocks. “One shot is enough for three weeks or more recently for up to three months,” said Jockenhövel. The hormone is stored in the muscles and released continuously. “The athlete would have a testosterone tank in his butt.” The doctor explains that the dose can be set in such a way that the level is constantly in the upper third of the normal range. Other drivers would only have such high values ​​sporadically.

Lance Armstrong, who won the tour seven times, would have been a candidate for medically-based testosterone. The American had to have his right testicle completely removed in 1996 after cancer. After that, he suffered from a hormone deficiency. “Since 1996 I have chronically too little testosterone and I can’t do anything about it,” he said in the “Playboy” interview.

Armstrong, however, denied having made up for the lack of hormones by taking testosterone. A year ago, he claimed that he could only start spraying testosterone after his professional career had ended.
